Questionnaires
There are 4 questionnaires to complete on nutritionDay.
The ICU unit staff completes all questionnaires about nutritional care and the patients’ illnesses.
Patients are only asked 7 questions as interview.
NEW!!! In 2025 ICU questionnaires have been updated to better describe nutritional care of ICU patients.
NEW!!! try the ICU WEB-APP to collect and enter data at the same time. (Available in MAY2025)

1. Unit Sheet: asks for nutrition related organisational and structural information on the ward. (1 page per unit - Duration 5min)
2. Patient Medical Information - ICU Admission Day: asks for patients status (gender/age/weight/height etc.), date of ICU admission; laboratory parameters on admission day. (2 pages - Duration 10min/patient)
3. Patient Medical Information - on nutritionDay: includes questions about treatment (insulin therapy/antibiotic treatment, etc.) and laboratory parameters on actual day (nDay). (2 pages - Duration 10min)
4. Patient Nutrition: comprises questions about medical nutritional therapy (enteral and parenteral nutrition, ONS). Number of days of parenteral/enteral feeding, planned amount of calories/day and the patients well being. (2 pages - Duration 10min)
ICU productlist:
60 day - Outcome Evaluation: re-assesses the patient's outcome 60 days after nutritionDay (discharged, still in hospital, transferred, death, surgery...). (Duration 5min per patient)
